If you'd like to fill out paperwork before arriving, the five forms below are downloadable. Otherwise we'll hand them to you at check-in.

We see you, fix it, send you home
Most visits wrap up in well under an hour. Your first prescription is free, filled before you leave.
WHAT TO BRING
Three simple things keep your visit quick and smooth.
Photo ID
A driver's license or any government-issued photo identification.
Insurance Card
Your current card so we can confirm your benefits at check-in.
Medication List
A list of the prescriptions and supplements you currently take.
FORMS (OPTIONAL)
Five PDFs, all optional. Fill them out at home if you would rather skip the clipboard time; otherwise we hand them to you at check-in.
